Update: Admissions in to SVIMS Paramedical required documents 2024-25
Application is common for all courses.
Single application is sufficient for all courses.
The candidates are advised to keep the
following documents / information handy before filling the application.
i)
Student Aadhaar Card.
ii)
AP EAPCET-2024 rank card (Soft copy
also) iii) AP EAPCET-2024 Hall Ticket.
iii)
Inter Marks Memo v) SSC Certificate vi)
Recent passport size Photo vii) Mobile number
iv)
Valid E-mail ID

Enter Aadhaar Number & AP
EAPCET-2024 Hall Ticket No, & Caste details. Pay the fee as mentioned
above. The data will be automatically captured i.e., Candidate name, Father
name, Mother name, Rank, Score secured etc., from Board of Intermediate or AP
EAPCET Further details about Grade/ Marks secured in intermediate or equivalent
exam shall be filled as per the certificate.

Other details like Candidate
particulars such as Date of Birth (as per SSC), Gender, Category, Special
category, Address, Study district details (Class VI to Intermediate) shall be filled
manually as per the certificate.
Uploading of documents:
i.
Upload photograph and Signature in size
less than 100 KB each in jpg/ jpeg format (Mandatory)*.
ii.
Scanned documents of the following
(Size less than 1 MB in PDF format):
a)
AP EAPCET- 2024 Rank card
b)
Inter marks memo
c)
In case the candidate belongs to other
than OC category shall select caste i.e.
i)
BCA/BCB/BCC/BCD/BCE/SC/ST/EWS and
upload recently obtained Caste certificate issued through e– Seva / Mee-Seva
(Mandatory)*. (Soft copy also)
ii)
Only if applicable: Upload documents of the following (Size less than 1
MB in PDF format):
iii)
i) Special category certificate – If
the candidate belongs to any of the special category -
iv)
NCC, CAP, PWD (Physically handicapped),
TTD/ SVIMS , EWS (if applicable). For more details refer clause 9.3 & 9.7 /
Annexure V. (Mandatory)*.
v)
ii) Residence certificate or equivalent
for 15% eligibility (if applicable): Documentary proof, if candidate belongs to
Un- Reserved i.e. studied outside the state of Andhra Pradesh. For more details
refer clause (8. 4).
vi)
Address for communication shall be
filled manually.
vii)
After successful completion of the
application & courses, priority selection, take a print out of both Fees
receipt & Application abstract for future reference.
viii)
The Application proforma (prototype and
Application printout (prototype) are available for reference – Annexure I &
II respectively.
Applications through online:
· Only eligible candidates as per the
regulations who desire to take admission into B.Sc(Nursing), B.P.T, B.Sc.
Paramedical (AHS) courses shall apply online and upload required scan copy of
original certificates as per the online application through internet. The
applications with incorrect information, invisible documents will not be
considered and automatically rejected.

The candidate is advised to fill the
online application form after going through the Prospectus by keeping all the
Certificates ready to enter the correct data and uploading of the scanned
copies of certificates/documents, photo and signature after payment of fee
through online.

The application can be submitted
through AP Online centres with the data and original certificates required.
·
The candidate can also fill the online
application form available in the Website of SVIMS.
·
The online application form can be
filled through any computer with
·
internet connection (home/internet
cafe/net center)
·
If it is found that the candidate has
furnished incorrect information or uploaded fake or forged documents, the
University will take action that deems fit as per the rules against the
candidate.
